Acute care hospitals, clinical liaisons, therapists, doctors, patients and families depend on our admissions staff and case managers for information, coordination and support. The admissions staff collect all medical and financial information pertinent for each patient. Case Managers coordinate with clinical liaisons, doctors and acute care hospitals to determine if a patient would benefit from acute rehabilitation services. Once a patient is admitted to the hospital the support staff helps the patient coordinate, on an ongoing basis, with their health care provider. Patients needs continue after they leave the hospital and the support staff work with providers and community organizations to help meet the projected equipment, financial and employment needs of our patients.
